Background Check Disclosure & Authorization

Your rights under the Fair Credit Reporting Act before a consumer report is requested about you

This document consists solely of this disclosure, as required by the federal Fair Credit Reporting Act (FCRA). A prospective or current employer using Hireqo LLC ("Hireqo") may obtain a "consumer report" and/or "investigative consumer report" about you from a consumer reporting agency for employment purposes.

Summary: A report about you may be requested. You must give written authorization before it happens. You have the right to see it, dispute it, and be notified before any adverse decision is made based on it.

1. What May Be Requested

Depending on the position and what the employer selects, one or more of the following may be requested about you:

  • Identity Verification — confirms your identity against government-issued ID
  • Basic Background Check — criminal history and public records search
  • Enhanced Background Check — expanded criminal history, civil records, and additional public records
  • FBI Fingerprint Coordination — fingerprint-based criminal history check coordinated through a channeling agency
  • Drug Test Booking — scheduling and results coordination for employer-required drug testing
  • Employment History Check — verification of prior employment
  • Education Verification — verification of degrees or credentials claimed
  • Driving Record Check — motor vehicle record, for positions that involve driving

These reports may be obtained at any time during your application or employment, for as long as you remain employed, to the extent permitted by law.

2. Your Authorization Is Required

No report described above will be requested until you affirmatively consent within the Hireqo platform. You may decline consent for any request; declining may affect your candidacy for a position that requires that check, but is not itself reported to any employer beyond the fact that you declined.

3. Your Rights Under the FCRA

  • You have the right to request a copy of any consumer report obtained about you.
  • You have the right to dispute the accuracy or completeness of any information in a report directly with the consumer reporting agency that furnished it.
  • You have the right to request a description of the scope and nature of any investigative consumer report.
  • Inaccurate or incomplete information must be corrected or deleted by the reporting agency.

4. Before Any Adverse Action Is Taken

If information in your report may lead an employer to take an adverse action against you (such as not hiring you, or termination), before that decision is finalized you will receive:

  • A pre-adverse action notice, a copy of the report, and a copy of "A Summary of Your Rights Under the Fair Credit Reporting Act"
  • At least five (5) business days to dispute the report before any final decision is made
  • A final adverse action notice if the employer proceeds, explaining your right to obtain another free copy of the report and to dispute it further with the reporting agency
